Περίληψη: Background: Moral distress is a common occurrence for neonatologists working in neonatal critical care. However, it remains a still partly unexplored concept among neonatologists. Despite a growing number of critically ill neonates, only a few studies related to moral distress as experienced by neonatologists have been published over the last years in Greece. Objectives/aims: The aim of this study was to define and identify barriers to and facilitators of moral distress based on the perceptions and experiences of neonatologists working in neonatal intensive care units (NICUs) in Greece. This pilot study was a part of a larger nationwide project. Methods: A prospective qualitative study using in-depth interviews with fifteen frontline neonatologists working in Greek NICUs was conducted. Interviews lasted 42 to 57 minutes. A conventional content analysis of data was conducted. Results: Participants defined moral distress broadly and relationally. A wide variation of definitions was displayed.  Organizational factors and the heigh uncertainty involved in many situations in the NICU context emerged as important facilitators of neonatologists’ moral distress. The overarching categories of reasons behind neonatologists’ moral distress were patient-related, family-related and physician-related factors. Furthermore, another finding emerged from the data analysis regarding participants' prior experiences of moral distress: moral distress is an essential component to providing medical treatment for neonates hospitalized in the NICUs. Conclusions: The present study contributes to better understanding moral distress as experienced by neonatologists working in Greek NICUs. Moral distress is conceived in the broad sense of the term (uncertainty distress) and relationally (in three dimensions). Neonatologists’ moral distress is conceptualized in the broad sense of the term, has relational character and is associated with multiple predisposing factors. A number of themes and subthemes were identified which, for the most part, were consistent with prior literature. However, we identified some nuances that are of practical importance. The results of this study support the need for establishing Institutional Ethics Committees in every healthcare setting and make neonatologist aware of them. Furthermore, the results of this study support the need to make professional psychological counselling available in every NICU.
