Περίληψη: Background: Idiopathic pulmonary fibrosis (IPF) is associated with lung cancer (LC);however, the mechanisms involved remain elusive. Telomerase reverse transcriptase (TERT), the catalytic subunit of telomerase enzyme, is frequently activated in LC. On the other hand, IPF is associated with mutations in TERT gene, leading to impaired function of telomerase and shorter telomeres. In the case of LC with concomitant fibrosis, the expression of TERT has not been studied. Aim: To detect the expression of TERT in cancer cells of patients with IPF-LC;to compare with LC without fibrosis. Patients and Methods: Tissue samples of 18 patients with IPF-LC (14 NSCLC and 4 SCLC) were retrieved for the Pathology Department of our center (Group A). For each case, a sex- and age-matched control with LC, without IPF, was assigned (Group B). The expression of TERT was studied with immunohistochemistry. The evaluation of the stain was performed by two pathologists. The intensity of the stain was additionally quantified with ImageJ program, as the mean grey value in color deconvoluted images. Results: All 36 samples demonstrated positive TERT expression. The percentage of positive cancer cells did not differ significantly between the groups (mean percentage: 90.3%vs86.7%). When comparing the stain intensity between the groups, Group A demonstrated significantly higher nuclear intensity (mean values: 0.64vs0.45, p<0.05), but significantly lower cytoplasmic intensity (mean values: 0.09vs0.21, p<0.05), indicating different intracellular localization of the protein between the groups. Conclusion: TERT was highly expressed in cancer tissues of patients with IPF-LC. Differential localization of the stain is reported, requiring further research.
